To REGISTER for counselling please submit the following form.  You will be contacted soon to make arrangements. 

NOTE:  If your call is urgent send this Registration Form to CMR and go to Emergency at your local hospital.  CMR will follow-up with you.

By using this program your Bargaining Unit Extended Medical Benefits Plan covers the FULL cost of services of a Registered Psychologist or Clinical Social Worker for up to 10 visits per covered person per benefit year - July 1 to June 30.   CMR invoices the Blue Cross Plan, so no claim forms to complete.  

If you do not have coverage with the Bargaining Unit Extended Medical Benefits Plan, counselling services are still available to you and your family at your expense at the REDUCED RATE of $105.00 per visit. You will pay the counsellor the full fee of $105.00 per visit and be issued a receipt.  If you or your spouse has coverage with another benefit plan you may be able to claim reimbursement.   (The standard rate for psychological services in Alberta is $160.00 for individuals and $170.00 for couples and families – Alberta Psychological Association.) 

NOTE:  You will pay a 1 hour fee of $95.00 for cancelled appointments or no-shows, unless you give the counsellor 24 hours notice.  The Bargaining Unit Extended Medical Benefits Plan will NOT pay for cancellations or no-shows. 

Once CMR receives the following information the counsellor assigned to work with you will contact you to arrange the first visit:

Names, birthdates, Blue Cross ID numbers for all who will be participating in the counselling

Confirm you are covered by the Bargaining Unit Extended Medical Benefits Plan - yes/no
